e2 education emPowers Scholarship Application

Application Deadline: March 15, 11:59 p.m.

This scholarship has been established by Mosaic Life Care Foundation to provide financial assistance to past participants of emPowerU programs. The intent is to honor a former emPowerU participant who has demonstrated a commitment to improving the quality of life within the region. This individual exhibits emPowerU ideals of impacting real-life challenges within their school, community or region by collaborating with others to bring about positive change.

Mosaic Life Care Foundation is a 501(c)(3) public charity dedicated to championing education and empowering people to build healthy and thriving communities.

Accepted and enrolled in a post-secondary education program (technical school, career center, community college or university); minimum of 12 hours per semester.
Participated in emPowerU program.
Maintain a minimum 2.5 GPA on a 4.0 scale.
Provide two current letters of recommendation.

Upload two current letters of recommendation. Each letter must include date of letter, the author’s name, relationship to applicant, professional title of employment and employment place.
