Mosaic Life Care Foundation and Mosaic Foundation – Albany are non-profit 501(c)(3) corporations organized under the laws of the State of Missouri. As such, the Foundation serves as the central repository for receiving and distributing philanthropic gifts on behalf of Mosaic Health System, Mosaic Life Care Foundation and Mosaic Foundation – Albany.

How do I include Mosaic Foundation – Albany in my will or trust?

In the online form, please select the “in honor of” or “in memory of.” Alternatively, if mailing a check, please make it payable to Mosaic  Foundation – Albany and accompany with a note stating in whose honor or memory you are giving the gift. Who determines how my gift is used?

Unless your gift is designated for a specific purpose when made, your gift will be used toward the area of greatest need at the time of use, determined by Foundation leadership.
