Auxiliary Fund Request

The Mosaic Life Care Auxiliary, an organization of caring volunteers, in cooperation with Mosaic Life Care, promotes community goodwill and provides support to enhance healthcare for our patients and their families.

1. The Auxiliary's fund request committee will review your submission and will ask you to present to the committee. If approved by the committee, you will also be asked to present tot he Auxiliary Board of Directors. The requests are voted on during board meetings and funds are released immediately following the meeting. Meetings are held every other month.

2. Fund requests in the amount of $5,000 or more need to be submitted through the Mosaic capital funds request process prior to submitting to the Auxiliary.

3. Emergency funding needs (without capital request submission) will be determined as needed.
