  • MOSAIC LIFE CARE AUXILIARY MISSION

    The Mosaic Life Care Auxiliary, an organization of caring volunteers, in cooperation with Mosaic Life Care, promotes community goodwill and provides support to enhance healthcare for our patients and their families.
  • PURPOSE

    The Mosaic Life Care Auxiliary awards scholarships to individuals pursuing a baccalaureate degree, who have demonstrated excellent leadership skills, academic achievement and community involvement. Students must meet minimum requirements. The Auxiliary Scholarship committee will make final selections.
  • ELIGIBILITY

    Applicants must be a high school senior or currently enrolled in undergraduate studies.

    High school senior applicants must provide documentation of acceptance into a college or university.
  • CRITERIA

    Applicant must have volunteered at Mosaic Life Care between July 1, 2024 and March 1, 2025 or be a Hillyard Technical Center senior enrolled in a Health Services program.

  • REFERENCE LETTERS

    Please upload two letters of recommendation; one of which is from the area in which you volunteer/volunteered or interned. Letter format must include authors name, title, company, applicant’s name, and relationship to applicant.
  • AWARD

    Each selected recipient will be notified of the $1,500 scholarship.

    Funds will be sent to the recipient's educational institution.

    This scholarship is available for first time applicants and is eligible for reapplication from previous award winners upon qualifications and continued volunteer service. NOTE: First time applicants will be given priority over any second time applications for renewals. A total of six scholarships are available for presentation.
